Normal Prostate Volume

Normal: 25-30 cc

Prostatomegaly Classification:

  • Mild: 30-50 cc
  • Moderate: 50-80 cc
  • Marked: >80 cc

PSA Density Cutoffs

<0.09: Low risk - unlikely clinically significant cancer (NPV 96%)

0.09-0.15: Intermediate - clinical correlation advised

โ‰ฅ0.15: Consider biopsy for clinically significant cancer

0.19-0.20: Optimal sensitivity/specificity balance

PI-RADS 3 Management

For patients with PI-RADS 3 lesions on mpMRI, PSA density โ‰ฅ0.15 ng/mL/cc warrants biopsy consideration, with positive predictive values approaching 60% for clinically significant disease.

Measurement Technique

According to ACR PI-RADS 2.1 guidelines, AP and TR measurements should be obtained on the sagittal plane for maximum accuracy.
